Jamil 2e436f7f77 chore(portal): Fix policy creation for conditions disabled; tweak design (#5301) 
- Fixes policy creation when `policy_conditions` is disabled
- Updates design so that items are a little more aligned and text has
more / consistent spacing around.

Test is failing though. Before sinking more time into this I wanted to
open this PR to get @AndrewDryga's input.

Web

To start your Phoenix server:

  • Run mix setup to install and setup dependencies
  • Start Phoenix endpoint with mix phx.server or inside IEx with iex -S mix phx.server

Now you can visit localhost:4000 from your browser.
