Reactor Scram 4104d679cd refactor(windows): Add context to errors, add SAFETY comments, update TODOs (#3517 )

The only semantic changes are:
- Add context to Windows errors
- Refactor some `bail!`'s that could be `context`'s

The rest is updating comments:
- Add `SAFETY: TODO` for unmarked unsafe blocks
- Elaborate on existing SAFETY comments 
- Close completed TODOs
- Link in Github issues for open TODOs
- Mark invariants or inter-dependencies between files that aren't
captured by tests or types yet
