c3593f868c Lavd: disable explicit codec thread settings 
It doesn't seem to have an impact and causes some missbehaviour with
H.264 and current version of libavcodec in Ubuntu.

e28eb70f4c main.c: enable signal handling for MSW 
Presumably doesn't work under MSys (program terminates after signal
handler). On the other hand, under default Command Prompt it behaves
correctly.

44b64b24f5 Lavc: moved to an older API 
AVFrame::width AVFrame::height and AVFrame::format was introduced with
lavc 53.31.0 so we are now compatible with prior versions.

cdc134d0ad Audio: show help when requested 
Do show help also if user asks for sth like '-r alsa:help', in addition
to '-r help'. The former one, of course, lists only ALSA playback devices.

d8c89a4e53 Remove lock from PortAudio modules 
This kind of lock (FS) is probably not feasible under MS Windows. From
code review it doesn't seem that the critical sections could overlap,
either.
