Fix docs README.md links to kubernetes.github.io
...
kubernetes.github.io uses index not README for it's base dir. So we were
linking to the wrong place.

All .md files now are only a pointer to where they likely are on the new
site.
All other files are untouched.

- extract the portion related to multi-cluster operation into a new multi-cluster.md doc
- merge the remainder (that was basically high-level troubleshooting advice) into cluster-troubleshooting.md
