Remove code related to DH_SWAP_BY_POINTERS and
DH_REPLACE_LAST_THREE_LADDERSTEPS_WITH_DOUBLINGS, as they are
not really worth it (code size increase of 60/100 bytes respectively
for 1000/500 us time gain).
